• Why we need your name and date of birth: We use these details to verify your identity and make sure you are the right age for our services. This provides us with the legal basis to offer you health and medical services, and to arrange the right support for you.
• Who we share your information with: To keep you and others safe, and to make sure you get the best possible care, we might need to share your details. This would only ever be with your doctor, National Health Service bodies, your next of kin, statutory services, or our trusted mental health partners. We also use fully anonymised data to monitor who is using our services and provide statistics to the people who fund us.
• How long we keep your information: Your form submission is kept on our secure server for thirty days. To keep your information safe, we completely delete all form submissions every single month.
