The Rehabilitation Team supports people with complex mental health difficulties to achieve their goals and gain the skills and confidence to live as independently as possible.

The Open Mental Health Rehabilitation Team helps those with complex mental health needs work toward independent living and achieving their goals. Established in April 2023 as part of NHS Somerset’s mental health transformation, the service mainly operates in the community to help avoid hospital admissions.
Holistic vision
The service provides longer-term, holistic support to support people with daily living activities including physical health and support to find housing.
Working together, a team of dedicated multidisciplinary professionals walk alongside people at their most vulnerable. Our multi disciplinary team consists of a Consultant Psychiatrist, Psychologist, Mental Health Nurses, Occupational Therapist, and Rehabilitation and Wellbeing Workers.
Community Rehab is a shining example of what true collaboration can achieve. Bringing together diverse expertise to deliver compassionate, person-centred care. There is real strength in working together, and it’s that unity that drives meaningful change in the lives of those we support.

Linked with services like Open Mental Health, Recovery College, and partner groups outside the NHS (housing and voluntary sector).
From its inception, the service has been designed and shaped with the support of people with direct lived experience of living with long-term mental health difficulties.

Referrals
Currently, only professionals from Somerset Foundation Trust can make referrals; self-referrals and external referrals are not accepted. If interested, speak to your family or keyworker who can initiate a referral if appropriate.
Next Steps After Submission
Referrals are discussed in a joint meeting involving Willow Ward and the Rehabilitation team.If the referral is deemed appropriate, an assessment will be arranged within four weeks.
If the referral is not suitable, the referring professional will be notified and provided with recommendations for alternative options.
